The organization implements evidence-based development and humanitarian programs across multiple sectors including Health and Nutrition (HIV/AIDS, TB, SRH, MNCH), Education, Social Mobilization and Livelihoods, Migration and Trafficking, Child Protection and Development, Human Rights and Good Governance, Humanitarian Emergency Response, and Environment and Climate Change.
MSD works in close collaboration with government institutions, community structures, and development partners to promote equity, resilience, and inclusive growth.
Position: Community Health Worker
Job Summary
The Community Health Worker will be responsible for engaging community members in planning, implementation, review, and identification of new HIV cases, and linking them to chronic care and treatment support services. The role focuses on community-based HIV testing, care, treatment adherence, and referral services.
Major Duties and Responsibilities
1. Targeted Community-Based HIV Testing Services
- Provide community-based HIV testing through mobile, index, and home-based approaches
- Conduct risk assessment, education on risk reduction, and condom promotion and distribution
- Create targeted demand for HIV testing services
- Identify and refer cases from health facilities in collaboration with providers or case managers
- Facilitate community-based care and support for non-stable patients to reduce health facility burden
- Mentor and supervise Community Resource Persons (CRPs)
- Counsel high-risk family members and conduct testing based on client preference
- Conduct assisted partner notification and community-based testing
2. Care and Support
- Conduct symptomatic screening for TB, STIs, and major depression
- Provide nutritional assessment, counseling, and stigma-reduction behavioral interventions
- Promote WASH practices and malaria prevention
- Provide education on family planning
3. Care and Treatment
- Support HIV-positive clients with disclosure and ART literacy
- Conduct home visits to strengthen treatment adherence
- Support tracking and re-engagement of lost-to-follow-up cases
- Identify and refer clients experiencing major drug side effects
Job Requirements
Educational Qualifications
- Diploma or BSc in Nursing, Health Officer, or other related health fields
Experience
- Minimum of 4 years relevant experience for Diploma holders
- Minimum of 2 years relevant experience for BSc holders
- Certified and experienced in HIV Counseling and Testing
Required Skills
- Strong health service delivery and community engagement skills
